Sports Physicals

Sports Physicals will be required for all middle and high school athletes. These physicals must be on file prior to practice. The IHSAA physical exam form is available at the link below. Once completed, you may bring in your paper form in person, or you may email a digital copy to Athletic Director, Brandon Ecker.
